Health Plans and heart checks.
  • People with one or more long term health problems benefit from a yearly appointment with one of our practice nurses and their regular GP.  This is an opportunity to ensure everything is being done to prevent a flare of these conditions and keep you healthy.
  • These appointments will go for 30-45 minutes and are a chance to get any scripts, blood tests or referrals that are needed.
  • At these sessions we want to work with you to come up with three or four goals for your health that you want to work with us to achieve.  It is useful to think about this before your appointment.
  • In creating these goals, we want them to be SMART.  Specific, Measurable, Achievable, Realistic and Timing
  • Your eligibility for a home medication review (Link to section) is assessed at these appointments
Wellbeing assessments – particularly for people over 75.
  • Some people find these a bit intrusive but we believe that these are good opportunity to pick up problems early and prevent deterioration.  The aim of these wellness assessments is to keep you healthy, living at home and enjoying your life.   The appointments usually go for over an hour and involve time with our nurse and then your GP will join to finish.  This is also a chance to get any scripts, blood tests or referrals that are needed.

Vaccinations
  • Our nurses are able to independently administer all vaccinations under the National Immunisation Program.  This includes vaccination for children, adolescents, flu vaccinations, pneumonia vaccinations and shingles vaccinations.
  • Our nurses can administer travel vaccinations, but your specific requirements need to be discussed with your GP first.  Click here for more information about travel vaccination
